2008-2009 Sutherland School Student Directory

Dear Parent/Guardian:

The 2008–09 Student Directory is under way! Please complete and return this form no later than Tuesday, September 16, 2008. This will be your only opportunity for your child to be included in the directory. Participation is optional. Each family will receive one directory.

If your child was included in last year’s directory, he or she will automatically be included in this year’s directory, unless you indicate otherwise. If your child was listed in last year’s directory, only complete this form if there is updated or corrected information. Please complete one form for each child.
